Most states have CMP Reinvestment Programs that provide funds to Certified Nursing Homes through an application process, which can vary by state and is often time-consuming.
Reading2Connect® offers a free service to help.
We complete the CMP application on your behalf, so you can apply for funding to bring the Reading2Connect LTC Program to your nursing home. See steps below.

The Nursing Home Completes the "Nursing Home Profile"

The nursing home or senior care company decides whether to bring the Reading2Connect® LTC Program to their facility or facilities.

Reading2Connect Completes the Grant Application

After reviewing the Nursing Home Profile and confirming eligibility, Reading2Connect® completes the CMP application at no cost.

The completed application is sent to the nursing home for review.

Once approved, the nursing home submits the application to the appropriate state agency.

The nursing home is the applicant and grantee, responsible for communication with CMP review agencies during the review process.

If approved, the nursing home receives funding to implement the Reading2Connect® LTC Program.

Over the next year, the nursing home reports outcomes to the state, as required.
